Who We Are...
We are a NEW, independent Targeted Case Management agency!
OUR TEAM CARES!
When you work with us, you will experience a knowledgeable and supportive team with diverse professional experiences focused on serving children and adults in the community who have intellectual and/or developmental disabilities.
All of our case managers have college degrees. We have case managers with backgrounds in education, special education, and mental/behavioral health. Because of that, we work collaboratively with community providers, agencies, organizations and state departments.
Most of all, we are passionate about supporting each individual in ways that best align with their person-centered plan.
What We Believe:
We are conflict-free advocates who help support the goals and lifestyle wishes of a person who receives services through the intellectual and developmental disability waiver. We believe in the individual rights of a person and provide support as needed to help a person communicate their preferences, abilities and support needs to live their best life.
We are experienced navigators of local, state and federal systems and process to access supports.

Our Managing Director
Sarah Otto, MPA, M. Ed.
- B.S. – Special Education, University of Maryland
- M.Ed. – Education, University of Maryland
- M.P.A. – Master’s Degree in Public Administration, Nonprofit Management, University of Maryland
Sarah Otto has over 20 years of experience working with individuals with disabilities/special needs. In fact, she taught in various public-school settings in both Kansas and Missouri. Yet, she found her passion serving as a Targeted Case Manager for individuals with Intellectual and Developmental Disabilities (I/DD) in Johnson County, Kansas, over the last 8 years.
Sarah is originally from Connecticut and attended college at the University of Maryland. When she’s not at work, she can be found enjoying a new restaurant in Kansas City with her family or serving on the Board of Directors for Camp Encourage.
Among her many talents, Sarah is known to have a “green thumb” and enjoys gardening. She also enjoys researching genealogy and spending time with her husband and two daughters.
