What is Targeted Case Management?
Targeted Case Management assists a person (and their caregivers/support network) with identifying, developing, selecting, obtaining, coordinating, utilizing and monitoring paid services and natural supports.
The goal is to enhance the individual’s independence, integration, productivity and community inclusion that aligns with the person’s needs and lifestyle preferences. These preferences are outlined in the person-centered support plan.

All of our case managers have college degrees. We have case managers with backgrounds in education, special education, and mental/behavioral health. Because of that, we work collaboratively with community providers, agencies, organizations and state departments.
Most of all, we are passionate about supporting each individual in ways that best align with their person-centered plan.

The Services We Provide Include:
Targeted Case Management is a Medicaid service in Kansas designed to help specific groups of individuals manage and coordinate their healthcare and support services. The primary goal of TCM is to ensure that individuals receive the appropriate services to meet their unique needs, thereby improving their overall health and quality of life.
To that end, we offer the following:
Assessment
Person-Centered Support Plan Development
Referral to services support coordination
Monitoring and follow-up of ongoing activities and services
Assisting with transition and portability throughout the lifespan

We are conflict-free advocates who help support the goals and lifestyle wishes of a person who receives services through the intellectual and developmental disability waiver. We believe in the individual rights of a person and provide support as needed to help a person communicate their preferences, abilities and support needs to live their best life.
We are experienced navigators of local, state and federal systems and process to access supports.

Payment Options

- KanCare for IDD clients who are qualified for Title 19

- KanCare for IDD clients who are qualified for Title 19

BILLING
We bill Medicaid for our services. A person using our services must be eligible for IDD waiver (intake through their local CDDO); and Medicaid eligible to receive the service.
PRIVATE PAY
In an effort to provide increased access, we do offer Private Pay options for individuals and families who do not yet qualify for HCBS IDD waiver services or financially qualify for Medicaid Title XIX.
